Apart from the obvious case of wrong answer, an endless CAPTCHA cycle can be caused by the browser changing transmitted details at each page load. Add-on aimed at preventing any type of fingerprinting will cause you enter such endless cycles, especially when in synergy with IP addresses not assigned to residential ISPs. I had this with ultra-hardened browser configurations, or with many "privacy" addons enabled, which is exactly why I don't ultra-harden it anymore and keep the number of addons low.
